What is a Case Manager?
We are so excited that it’s October! Why, you ask, Halloween, the cooler temps, fall festivities, nope we are excited to celebrate CASE MANAGERS!
You read that correctly, CASE MANAGERS- the unsung heroes in the medical system who help so many people. The Case Management Society of America defines case management as, “Case Management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote patient safety, quality of care, and cost-effective outcomes. Case management serves as a means for achieving client wellness and autonomy through advocacy, communication, education, identification of service resources and service facilitation. Case managers are helpers. They are constantly giving and deserve to be recognized for their commitment to helping others, Next week is Case Manager Appreciation Week, and we plan to visit and deliver breakfast or lunch to as many case managers as we can!
